Pathways to Support Services

Depending on the type of service you require your pathway to a service provider will vary, read the guiding outlines below.

Support of 1-2 hours/week required
ie gardening, home cleaning, clothes washing, showering assistance This level of support can be provided by a Private Provider and some Home Care Service Providers who receive government funding under ‘Home Assist’ agreements. You will not need any formal Health Assessment or Centrelink Asset Assessment.

  • Contact your local Council for Service Provider information (some may offer home care services)
  • Web search ‘low level home care in ‘insert your town/suburb/region name’
  • Check local newspapers for service providers
Home based assistance required for more than 3 hours / week and may be up to 15 hours/week

(termed level 1-4 depending on degree of assistance required)

This level of support can be provided by a Private Home Care Agency or a Government Assisted Home Care Agency. You can make enquiries to Service Providers yourself. Private Services will cost more than Government Assisted. For Government Assisted Services you will need to have an ‘ACCR’ Assessment completed by an Aged Care Assessment team member stating approval for ‘home care’, ‘respite’ or ‘transitional care’ PLUS a Centrelink ‘Assets Assessment’ completed by Centrelink.

  • Contact Service Providers and arrange a home visit appointment to discuss your support plan.
Higher hours or 2 person care levels of support required / Aged Care Facility Support required
There are ‘Private-for-profit’ and ‘Private-non-profit’ Residential Care Agencies. Both receive Government subsidies to cover basic care costs. The non-profit organisations follow minimum/maximum charge expectations set by the government. For-profit organisations can charge more and usually provide a greater number of options of service. You will need to have an ‘ACCR’ Assessment completed by an Aged Care Assessment team member stating approval for residential care’ PLUS a Centrelink ‘Assets Assessment’ completed by Centrelink BEFORE any organisation can offer you a suite.
  • Complete the ‘Health Care Needs list’ in preparation for Aged Care Assessment questions
  • Develop a list of what your priorities, needs and wishes are in relation to what you expect to receive from your preferred residential care provider.

  • Choose 2 or 3 service providers and make appointments with each service to discuss your priorities, needs and wishes. When you visit the service take along the ‘Aged Care Facility Evaluation Checklist’ and use it as a guide for questions to raise and observations to make whilst you are there.
  • Complete the ‘Health Care Needs list’ to describe fully the levels of care support required by the Residential Care Facility. Take a copy for each organisation you visit and ask if they are able to offer these levels of care. Check if there are extra personal charges for any of these levels of care.
  • Residential Care Service Providers will offer you a ‘Contract or Agreement of Service’. It is recommended that your legal or financial advisor/agent read over this document for you. Accommodation options and services may vary within the one facility. Clarify the area/room that is being offered. Ensure the facility offers ‘aging on site’ so that your loved one will be catered for by that organisation no matter how their health care needs change.
  • Legally you have up to 28 days after admission to sign the contract/agreement.
