Home Care Options Checklist

Use this when considering what might be of help to you when living at home, what might prevent hospitalisations, improve your life experience and enable you to continue to live at home.
Take the completed ‘Health Care Needs List’, ACCR assessment completed by the ACAT assessors (only required if needing more than 2-3 hours of assistance each week in total) and your Centrelink Asset Assessment to all appointments with potential Service Providers.

Health Care Needs list

When you approach service providers they will ask you many questions about your current independence and healthcare issues. If you complete this document before any appointments it will ensure you don’t forget anything and that the plans you make together are based on what you need.

Aged Care Facility Evaluation Checklist

When you or a family member are about to visit an Aged Care Facility to see whether it fits your needs and expectations, take this along with you as a guide.

Charter of Rights & Responsibilities - Home Care

Outlines your rights as a recipient of home based services and can be used to ensure people are respectful and considerate of your rights.

Charter of Rights - Residential Care

Outlines your rights as a consumer and can be used to assess how service providers interact with or support you when you live in a Residential Care Facility.

Advance Care Directives

A document you can complete that legally states who youo nominate to represent you in health decisions if/when you are unable to do so. You can document legally binding directives for others to adhere to.

Aged Care Accreditation standards

Developed to clearly state the expectations you can have of the expected quality of business and healthcare service provision by Residential Care Providers. Use this when evaluating the quality of support you receive or can ask about.
